Participation in NAFA Soccer Event

Waiver / Release

LIABILITY RELEASE FORM

In consideration of my  participation for the above  mentioned soccer game, I hereby expressly waive, release, and discharge any and all claims for damages for death, personal injury, or property that I may have, or that may hereafter accrue to me, as a result of participation in the said event.  This release is intended to discharge in advance the organizers, the city of  Tempe, its officers, employees, or agents from any liability, even though that liability may arise out of negligence or carelessness of the persons or entities mentioned above.  It is understood that some recreational activities involve an element of risk or danger of accidents, and knowing those risks, I hereby assume those risks.  It is further understood and agreed that this waiver, release, and assumption is to be binding on my heirs and assigns as well.
